How to Exchange a Driver’s License for an ID Card
Driving retirement is tough! Add in trying to figure out how to turn a license in for an identification card can leave you confused, frustrated and overwhelmed. frustrations at the DMV can just make the whole process worse.
If it is time to help mom or dad turn in their driver’s license for an ID card, skip the lines and use the NEW updated PennDOT Form.
Form to surrender a license
Please note this form needs to be completed by the driver.
So start by printing the form.
Next help mom or dad complete the top portion of the form.
Then under Letter C, Item 2, check “CURRENTLY HOLD A PA DRIVER’S LICENSE/PERMIT AND AM APPLYING FOR A NON-DRIVER IDENTIFICATION CARD FOR THE FOLLOWING REASON”
Next, select, “I am surrendering my driving privilege for health reasons that may affect my ability to safely operate a motor vehicle. I understand that my license will not be reissued until I successfully complete the appropriate examination. (If you have not already surrendered your Driver’s License/ Learner’s Permit, please attach it to this application.)”
Then under Item 2,
